Follow-Through On Discharge Orders For Medical Teams, Home Health Agencies, Hospital Social Workers, & Case Managers With Out-Of-State Situations.
Scheduling Medical Appointments For A 3-Month Duration For:
- If Client has a PCP, then schedule the visit
- If the Client does not have a PCP, the Care Manager will assist with locating one.
